Shipment of the Remains of Deceased Persons to Nigeria

In the event of the need to transport the remains of a deceased person to Nigeria, the following documents are required for sighting and authentication by the High Commission:

- Photocopy of the deceaseds passport.
- Certificate of Death from the Health Department
- Medical Certificate of Death ( Form 16); issued by the appropriate Canadian authorities
- Burial Permit (Where applicable)
- Statement of Death ( Form 15); issued by the appropriate Canadian authorities
- Letter of indemnity from the Funeral Home which confirms that the remains will be shipped in an internationally acceptable form.
- A Pre-paid self addressed Priority Post Courier envelope for the return of the authenticated documents
- A fee of CAD $10.00 per each copy of the document, payable in money order to the Nigeria High Commission as processing fee.


CERTIFICATION OF DOCUMENTS

The original document(s) to be accompanied with the following:

- Two Photostat copies of each of the original document page(s).
- Evidence of the identity of applicant. e.g. Photocopy of passport with names that match those on the certificates.
- One passport-sized photograph
- Processing fee payable in Money Order to Nigeria High Commission, Ottawa is CAD$10.00 per original certificate.
- Send all the above to the High Commission & enclose a self-addressed return envelope prepaid by post express or priority post.

Note:

Original document(s) will be returned with the certified copy (copies).
Processing takes at least three working days.
Non compliance with the requirements may delay early processing.
